Discharge Planning Makes a Difference
Anecdotal update, worthy of a post: Mrs. J. (name has been protected) was so happy with the assistance she received from the Discharge Planner at Saint Francis Hospital in San Francisco. They set up all the services she needed to continue her recovery post surgery.
